Job Description:
A company is looking for a Sr. Behavioral Health Case Manager -
DSNP.
Key Responsibilities
Act as a subject matter expert to support clinical case management
staff
Manage care management task lists and work assignments while
monitoring performance metrics
Collaborate with case management leadership to develop strategies
for achieving goals
Required Qualifications
RN or LCSW with 3 years of clinical and/or case management
experience
Applicable licensure for Behavioral Health roles with a minimum of
3 years of clinical experience
Direct experience in case management with a record of meeting or
exceeding metrics
Proven expertise in case management functions and processes
Valid nursing or relevant clinical license within the US, with
application for NC license started
